2010 Certification Exams
Application deadlines – applications will be accepted in the PSTI office until 5:00 pm on the dates indicated. All applications must be completed in ink and be completely filled out, have all required supporting documentation and signatures. Certification fee is due with the application. Checks should be made payable to Luzerne CCC, PSTI and mailed to:

Refunds, less 20% of the application fee, will be given upon written request (e-mail acceptable) until 12 noon the day prior to the written exam. Refund requests will not be honored after that time.
Applications received the day of the deadline and having errors or missing information will be rejected and candidate will need to select another date for certification.

Certification Evaluator Workshop-Refresher
